Xian (冼) is a variant of Xi (洗), which originally meant "to wash one's feet." The character is composed of the radicals for Water (水) and Before/First (先). In Oracle Bone Script, the character depicts a "foot" (Zhi 止) above a "person" (Ren 人). Vessels used for ceremonial washing or ablution were specifically referred to as Xian.

The Xian surname is derived from the Xi (洗) surname and has a singular, distinct lineage. It originates from the ancient ethnic minority groups (often referred to as the Gaoliang people) in Southern China. According to the Xingshi Xunyuan (Origin of Surnames): "In Nanhai and Panyu (modern-day Guangdong), the Xian surname is prevalent; it is the surname of the tribal chieftains of the Gaoliang region."
